Licensed Experts
The Passages Hospice team includes experts in medicine, nursing, social work, chaplaincy and bereavement. All services are provided under the direction of a physician and/or the Hospice Medical Director.
Nurse Case Manager
- Coordinate your care and handle nursing needs
- Prevent and relieve pain and other symptoms
- Educate patient and family on health issues and signs of dying
- Supervise CNAs who provide personal care to patient
Certified Nursing Assistant (CNA)
- Provide personal services such as bathing, dressing, and eating
- Report any changes in condition to the hospice team
- Ensure that patient is comfortable and clean
- Personalize care to the patient’s condition and needs
Medical Social Worker
- Listen empathetically to feelings and emotions of the patient and family
- Determine patient and family priorities
- Assist in helping the family develop healthy coping skills
- Educate patients and families about advance directives
- Investigate and refer to community resources (as necessary)
- Assist with funeral planning
Chaplain
- Provide spiritual care for the patient and family
- Help patient and family cope with spiritual issues
- Provide regular, on-call and crisis visits
- Contact patient’s minister for additional support, if desired
- Advise without imposing his or her own personal belief system
- Provide bereavement counseling
Medical Director
- Visit patient to evaluate their hospice qualification
- Perform assessment of patient pain control needs
- Make changes to medication, treatment, etc. in the patient’s plan of care
