Passages Patient Messages

Using our secure Patient Messages form to the right, you can send a private, confidential message to your loved one. All messages sent to Patients will be personally delivered by one of our Patient Care Providers at an appropriate time.

Your message will be handled with extra care and will not be printed or shared with anyone other than the Patient Care Professional and the Patient. All of our Patient Care Professionals have years of education and training regarding Patient Care and Confidentiality and will respect your privacy.

Please include the full name of your loved one and the Passages Hospice location or other facility where they receive care, and enter your message to be shared at the next appropriate time.
