The Right Time to Refer Your Resident
Ideally, the earlier an individual receives hospice care, the more opportunity there is to stabilize their medical condition and address other needs. Please consider referring your residents when they have a survival prognosis of six months or less, when they still having some quality of life without the possibility of a cure, when they have month-to-month health changes or when 10 percent weight loss is noted.
Receiving hospice care does not mean giving up hope or that death is imminent. Some patients actually improve and at times have been discharged from hospice care.
